Membership Guidelines
- Membership application is required to begin service.
- Initial visit requires a comprehensive exam and x-rays.
- Club members portion of the billing for services is due on the day of service.
- Membership discount does not apply to products.
- Bi-Annual preventative care required for membership.
- Additional discounts DO NOT apply.
- Care Credit is NOT allowed as payment toward premiums or services.
Membership Limitations
This program is a discount plan, not a dental insurance plan. This program is honored only at Haskell Family Dental.
This plan cannot be used for:
- Services in conjunction with any other dental plan.
- Injuries covered under an accident policy. (School, Home or Workers Compensation)
- Any treatment which lies outside the expertise of Dr. Haskell.
- Specialist referrals.
- Hospital charges or hospitalization of any kind.
- Dental care costs covered under an automobile medical policy.
